Medicaid Look-Back
Timeline.

See how timing and transfers can affect planning.

A timeline for asking better questions.

Move the marker to explore how far back a hypothetical transfer occurred. A transfer does not automatically create a penalty; applicable rules and exceptions require review.

At the application point

This point is inside the federal 60-month look-back window.

Federal law sets a 60-month look-back for transfers made on or after February 8, 2006 (42 U.S.C. § 1396p(c)(1)(B)(i)). It counts back from a set date, usually around when the person needs long-term care and applies.

California (Medi-Cal) differs. The California Department of Health Care Services says: “If you move into a nursing home, Medi-Cal will look at any assets you gave away in the 30 months before you entered the facility.” The same page says: “Transfers made before January 1, 2026, will not be counted.” Source: Asset Limit Frequently Asked Questions.

Other states: each state applies its own rules; check your state's page.

This is not an eligibility determination.
